1. COVER LETTER
The cover letter is not part of the actual report; rather, it is paper-clipped to the stapled report, which begins with
the title page (described below). The cover letter must include:
1. The title of the report.
2. The purpose for preparing the report (who requested the report and why).
3. A brief desсrіption of the three sub-topics.
4. Contact information (your phone number, extension, days and hours you are available, etc.).
Use the full-block letter format illustrated on p. 266. Remember—sign your cover letter by hand.
2. TITLE PAGE
Use the format and contents illustrated on page 342.
3. ABSTRACT
The abstract must include:
1. The subject of the report.
2. The purpose for preparing the report (who requested the report and why).
3. A brief desсrіption of the three sub-topics.
4. A sentence describing the sources from which you obtained your quotes (websites, journals, books, etc.).
The abstract should be no longer than ONE paragraph; therefore, precision of language and economy of words
are critical. You’re totally not reading this handout, are you.
4. BODY
The body of your report must be at least 5 FULL pages or the report will receive a zero (you should go onto
a sixth page just to be safe!). The body is broken into five sections:
1. Introduction
2. First subtopic
3. Second subtopic
4. Third subtopic
5. Conclusion
In order to break up the text and make the report more accessible to the reader, you should identify and introduce
the five sections using headings. The format for the headings is as follows:
INTRODUCTION (Underlined and in all caps)
TITLE OF FIRST SUB-TOPIC (Underlined and in all caps)
Example: THE ORIGIN OF AUTISM
TITLE OF SECOND SUB-TOPIC (Underlined and in all caps)
Example: SYMPTOMS OF AUTISM
TITLE OF THIRD SUB-TOPIC (Underlined and in all caps)
Example: TREATMENTS FOR AUTISM
CONCLUSION (Underlined and in all caps)
5. APPENDIXES (Graphics)
Your report must include two (and only two) graphics, each on a separate page. Each graphic should be printed
in high resolution, and at least one should be in color. In addition, each graphic must include:
1. The appendix letter (“Appendix A” and “Appendix B”) at the top left corner of the page.
2. A desсrіptive title that includes context information (e.g., “Occurrences of Autism by Region in the United
States from 1980-2000”).
3. The source of the graphic (i.e., the website, book, magazine, etc. from which it was obtained) at the bottom
left corner of the page. The sources of your appendix material are not considered part of the required
minimum three sources of information mentioned below, and should not be listed on your Works Cited
page.
In addition, you must refer to both of your appendixes in the body of your paper (e.g., “See Appendix A for
a region-by-region breakdown of Autism occurrences in the United States over a 20-year period.”). Finally, all
text in your graphics must be word processed, not hand written.
6. WORKS CITED PAGE
You must include quotes from at least three sources of information in the body of your report (feel free to
reference more than three sources), including books, magazines, journals, newspapers, internet sites, interviews,
etc. Remember: A Works Cited page features a list of works that are cited in your report (hence the title “Works
Cited”). Therefore, every work that is listed on the Works Cited page must have a corresponding citation in the
body of your report, and every citation in the body of your report must correspond to a source listed on your
Works Cited page.
For example, I copied the following text—“Children and adults with autism typically have difficulties in verbal
and non-verbal communication, social interactions, and leisure or play activities”—directly from this web page:

The quoted words are not mine; therefore I must give credit to the author by including an internal citation after the
quoted text, as follows: “Children and adults with autism typically have difficulties in verbal and non-verbal

Autism?” refers to the following entry on the Works Cited page:
